School Registration

New Student Registration

The following documents are required for enrollment:

1. Birth Certificate (required for Kindergarten and 1st Grade)
2. Immunization Certificate (Form #3231)
3. Vision/Hearing and Dental Certificate (Form #3300)
4. Social Security Number or signed waiver available from school
5. Proof of residency in the attendance zone
   (Examples include a non-contingent sales contract, lease, or deed. A lease or deed must be accompanied by at least one utility bill (gas, water, electricity) with the same address. A contingency contract is not acceptable. Call your local school if you live with a friend or family member and need information about verifying your residency.)

Kindergarten Registration

What to bring to registration:

1. Your child
2. Your child's birth certificate
3. Georgia certificate of immunization (State Form #3231)
4. Vision, hearing and dental screening forms (State form #3300)
5. Proof of residency in the attendance zone
   (Examples include a non-contingent sales contract, lease, or deed. A lease or deed must be accompanied by at least one utility bill (gas, water, electricity) with the same address. A contingency contract is not acceptable. Call your local school if you live with a friend or family member and need information about verifying your residency.)
   
6. Proof of custody or guardianship (if not the birth parent)
7. Your photo ID
8. Your child's social security number (or you may sign a waiver at the school)

Your child's registration is not complete, and he or she will not
be assigned a teacher or be permitted to attend school,
until all documents are on file at the school.
